Connecting Patients at Home with Their Health Care Team

Telehealth by Visiting Nurse Association, Serving Morris and Hunterdon Counties

VNA's Telehealth Program monitors a patient's vital signs and symptoms daily from the comfort of their home and transmits the information directly to their health care team.

Through Telehealth, VNA is able to provide personalized, individual care while allowing patients to live more independently with confidence. With Telehealth, VNA can provide better patient care, assist patients in avoiding hospitalizations and pinpoint the best time for a home visit.

Eligible patients are loaned, at no cost, a Home Monitoring Device that sends daily health information automatically and securely to VNA. Once received, the information is reviewed and the patient's home care team at VNA will adjusts the plan of care, as needed.

VNA has partnered with CardicomĀ®, reputable developer, manufacturer and solutions provider of clinical telehealth services, to provide patients with this service.

Easy to useā€¦

  • A clear, friendly voice prompts the patient walks them through their daily Health Check. Takes only minutes a day.
  • The patient's daily health information is automatically and securely sent over their phone line or the system's internal cellular modem to your health care team.
  • The patient's health data is analyzed and their health care team determines if a change in medication or treatment plan is necessary.
