Medication management is a major concern for older Americans. In fact, more than 87% of people over age 65 take multiple prescription medications and people over age 75 take an average of eight prescription drugs on a regular basis. The VNA provides one-on-one visiting nurse consultation programs and information about electronic medication management devices to help prevent dangerous health implications that can result from inadvertent misuse of prescription medications.
If you or a loved one is experiencing physical symptoms or forgetfulness that impedes medication compliance, Click here to request more information or call 1-800-WE VISIT.
Telehealth is an interactive system that monitors patient's vital signs and symptoms daily from the comfort of the patient's home. Eligible patients are loaned a Home Monitoring Device that sends daily health information automatically and securely to VNA. Once received, the health information is reviewed and we will make adjustments to the patient's plan of care, as needed.
This program was developed so that we can provide better patient care, assist in avoiding hospital re-admissions and pinpoint the best time for a home visit to occur. Through telehealth, we are able to provide personalized, individual care while patients are able to live more independently with confidence. Click here for additional information.
